Step 1:

Make sure you're signed in to the primary Google account you wish to connect the form with. Then create a copy of the template Google Form by clicking "Copy Google Form" below.


Step 2:

Feel free to change the form name, but do not edit or delete any of the required questions. Next navigate to the Responses tab and click "Link to Sheets" to create a new spreadsheet.


Step 3:

Return to the Google Form.

  1. Select the Publish button in the top right corner, click Manage, and share the Responder view link.
    Be sure to click Publish after clicking Done
  2. Under more options select "Get pre-filled link". Then enter dummy responses into all required question (marked with a red asterisks). Do not provide responses for non-required questions.
Note the responses do not need be real patient data, but try to provide a realistic example. Once you have entered responses into each of the form's required fields, click "Get link" at the bottom of the form. Then click "COPY LINK" to copy the link to your clipboard.

Step 4:

Paste the link from Step 3 into the input below.

Step 5:

Enter the GitHub account username you wish to use for hosting the digital form.

Step 6:

Fork a copy of the repository


Step 7:

Select the green button that says "I understand my workflows, go ahead and enable them" at the link below.

The following button will take you to the GitHub Pages settings.
Change the build and deployment source to GitHub Actions.


Step 8:

Click below to copy your generated site configuration.


Step 9:

Replace the contents of the config.json file with your copied configuration.
